The use of dental implants as a treatment for replacing missing teeth has been increasingly prevalent. Historical evidence suggests that the Mayans were among the first to create implants around 600 AD. Over time, this technique has evolved, offering numerous benefits to humanity. Regardless of the causes of tooth loss and the patient's age, it significantly impacts various aspects of an individual's life, including nutrition, psycho-emotional well-being, and social interactions. One of the primary consequences is the negative effect on self-esteem due to the aesthetic concerns associated with missing teeth. Additionally, pronunciation difficulties can lead to communication challenges, reducing the desire and ability to engage with others. Extensive research in the specialized literature explores the psychological impact of dental implant usage. It is widely acknowledged that the dental implant technique is complex and carries inherent risks and potential adverse effects. An illustrative case serves to underscore the significance of psychological considerations in dental implant treatment. In this instance, a 55-year-old woman attempted suicide by throwing herself down the stairs of her home after receiving dental implants in both sections of her oral cavity. The distress she experienced stemmed from the inability to consume anything beyond fluids and semisolid food for an extended period of eight months. This alarming incident underscores the importance of recognizing and addressing the psychological impact that dental implant procedures can have on vulnerable individuals.
Background: This research aimed to analyze the effect of experience and professional role on psychophysiological stress response in underwater evacuation training. Methods: We analyzed 36 participants (39.06±9.01 years), divided into two different groups; 17 crew members (38.6±7.2 years) and 19 medical members (39.5±10.5 years). modifications in the rating of perceived exertion, subjective stress perception, heart rate, blood oxygen saturation, cortical arousal, heart rate variability, spirometry, isometric hand strength, and short-term memory before and after underwater evacuation training were analyzed. Results: The maneuver produced a significant increase in SSP, RPE, IHS, FVC, and SatO2 at different moments of the intervention, being higher in crew members. Conclusions: We found that the underwater evacuation training produced an anticipatory anxiety response, and an increase in autonomous sympathetic nervous system modulation not affecting strength capacities, cortical arousal, and memory independently of the aircraft role (medical or crew).
Systemic lupus erythematosus is a multi-organ autoimmune disease of unknown etiology characterized by widespread inflammation and significant morbidity and mortality. Organ affection includes joint and cutaneous involvement, pulmonary, neurological, and cardiac problems, renal and hematological involvement as well as ocular comorbidities, both posterior but mostly anterior ocular involvement. Therefore, we aimed to evaluate dry eye involvement in lupus patients. A cross-sectional study was performed with the aid of a web-based, anonymous questionnaire that was distributed to both patients diagnosed with lupus erythematosus and to a control group. A total number of 123 patients diagnosed with lupus and 200 responders in the control group completed the questionnaire. Dry eye disease has already been diagnosed in 25 lupus patients (28.9%) compared to 44 (21.9%) individuals in the control group. Dry eye-related symptomatology was reported in 43 (34.60%) patients compared to 57 (28.35%) healthy responders. Upon statistical analysis, more symptoms were reported in the lupus group (p=0.01), in responders diagnosed with dry eye syndrome (p<0.01), and in responders using lubrication (p<0.01) but no association was obtained between the incidence of dry eye symptoms and the presence of the disease. The number of episodes of lupus reactivation since diagnosis was correlated with dry eye syndrome (p=0.15) and the use of lubricating tear drops (p<0.01). Dry eye symptoms and disease are more frequent in lupus patients and further research should be performed to understand the connection between these two disease entities.
Couvelaire syndrome is a rare complication of pregnancy involving the placenta and the uterine wall. It is caused by placental abruption, preeclampsia, or other disorders that compromise the blood supply to the placenta. Diagnosis is typically made through history, clinical evaluation, and ultrasound. This study aims to report a grave case of Couvelaire syndrome complemented by presenting a review of the relevant literature, to aid the meaning of a clinical path, as well as early and adequate diagnosis and treatment of Couvelaire syndrome, to prevent feto-maternal consequences. The treatment of Couvelaire syndrome depends on the severity of the condition and the well-being of both the mother and the fetus. If the condition is severe and poses a risk to the mother or fetus, immediate delivery may be recommended, mainly with urgent Cesarean section in general endotracheal anesthesia. If there is significant blood loss leading to a decrease in hemoglobin levels, blood transfusions may be necessary to restore normal blood volume and oxygen-carrying capacity. Part of the general treatment plan should also be the management of the underlying cause of placental abruption, for example, in cases of preeclampsia, medications are prescribed to control blood pressure and prevent seizures.
This paper focuses on the role of diffusion-weighted MRI of ovarian tumors as a potential biomarker to detect and characterize malignancy, and to improve the accuracy of preoperative diagnosis and postoperative follow-up to optimize therapeutic management. This is an original retrospective study of 76 patients with ovarian masses who underwent surgery between May 2014 and March 2016 at the "Dr Carol Davila" Central Military University Gynaecology Department. Patients underwent preoperative, 6-month, and 12-month postoperative evaluation of ovarian tumors using diffusion-weighted MRI. Due to the accuracy of this method using specific sequences in tissue characterization, magnetic resonance imaging can be used in the diagnostic work-up of complex adnexal masses.
Chronic pelvic pain syndrome (CPPS) presents a diagnostic challenge due to its multifactorial nature, often involving urological, gynecological, and psychological components. This study focuses on the effectiveness of prophylactic phytotherapy in preventing CPPS recurrence, aiming to mitigate symptoms and psychosocial impact. The study includes 74 patients following a six-month treatment regimen with Graminex, Seronoa Repens, Curcuma Longa, and Acediboswellic ke-to-acetyl-11. Symptom assessment employs the IPSS and NIH-CPSI questionnaires, conducted at 3, 6, 8, 9, and 12 months. Patients exhibit significant improvements in IPSS scores, with maintenance of treatment impact observed at 3, 6, and 9 months. Interruption of therapy leads to symptom recurrence, but upon resumption, responses are more effective. The third group, under observation for three months, reports positive outcomes. Both IPSS and NIH-CPSI scores show a consistent impact on urinary symptoms and pelvic pain, indicating the therapy's effectiveness. The study demonstrates a concordance in the positive effects of phytotherapy across different age groups, emphasizing its efficacy in alleviating urinary symptoms and pelvic pain in CPPS patients. Symptom remission or improvement in quality of life is reported, with a notable tropism for symptom reduction in the cold season. Patient adherence to treatment is high, and even after treatment interruption, resumed therapy yields significant improvements. The observed benefits include the absence of pelvic pain, dysuria, and enhanced sexual competence, contributing to an overall improvement in the quality of life for patients with CPPS.
With recent advances in the understanding of psoriatic disease, it is increasingly considered a systemic inflammatory condition rather than limited to the skin and joints. A variety of biologics are available today for the treatment of psoriasis, but with them, characteristics such as rapidity of onset, long-term efficacy, safety profile, and effects on comorbidities differ. We designed an observational, non-interventional, retrospective study of patients with severe psoriasis receiving biologic treatment with IL-17 inhibitors and aimed to investigate the correlations between etiopathogenic factors and the efficacy and persistence of these therapies in a group of psoriasis patients from Romania. Study results show that patients treated with ixekizumab had better persistence and high adherence compared to those treated with secukinumab. In this study, ixekizumab demonstrated lower risks of non-persistence, discontinuation, and switching and a higher likelihood of high adherence compared with secukinumab. This study contributes to the understanding of the persistence of anti-IL17 biologic therapies in psoriasis and the factors that may influence it.
Colorectal cancer is the third most common cancer worldwide and represents a significant major public health issue. Therefore, the interest in detecting the multiple factors implied in the development of cancer has increased recently. One of the factors could be the Helicobacter pylori infection, but a direct causal connection is missing. This gram-negative bacterium infects approximately 4 billion individuals globally and various conditions such as gastroduodenal ulcers or gastric adenocarcinomas can be induced by it. Some of the reported carcinogenic effects of Helicobacter pylori bacterium are hypergastrinemia, chronic inflammation, dysbiosis, and toxin production. Each of these elements involved in the pathogenesis has various mechanisms of action. Dysbiosis can occur due to various treatments for the eradication of the Helicobacter pylori infection. This bacterium also has a proinflammatory effect at the gastric level which can predispose to the development of dysplasia or even neoplasia. The toxins produced by this bacterium, such as the vacuolating cytotoxin A, promote inflammation. In this review, we discuss the possible connections between the Helicobacter pylori infection, through its diverse pathogenic mechanisms, and colorectal neoplasm. Prospective studies are necessary to determine whether there certainly is a direct link between the two conditions.
