Pharmacological evaluation of glomerulonephritis in Covid-19 patients

Acute kidney injury (AKI) and glomerulonephritis can be complications in COVID-19 patients, which is announced with widely incidence rates in different investigations and is determined to have a major effect on the prognosis of the disease. There are considerable variations in AKI and glomerulonephritis rates between other countries. The rate is generally lower than in Western Europe and the United States in China. Heterogeneity in different racial and ethnic lines can be a potential explanation. This investigation systematically reviews the scientific resources regarding AKI and glomerulonephritis among hospitalized COVID-19 patients. Both observational and interventional investigations(including case reports) with English full-text provide essential data, with no limitation in release or peer-review. More evidence is required to assess the AKI, especially glomerulonephritis in Covid-19 patients, for better management of Covid-19.

Outcomes of frozen embryo transfer in patients with and without ovarian hyperstimulation syndrome

Introduction: One of the methods utilized to treat infertility is the use of frozen embryos. This technique is particularly employed in patients with ovarian hyperstimulation syndrome (OHSS), which leads to heightened blood estrogen levels. The purpose of this study was to compare the percentage of pregnancy and abortion between patients who used the frozen embryo method due to OHSS and others who practiced the same method for other reasons. Materials and Methods: This retrospective, cohort study was conducted on a total of 338 patients who visited in vitro fertilization (IVF) section of Kowsar Hospital of Urmia-Iran to employ the frozen embryo method (May 2013 to December 2015). The patients were classified into two groups, i.e. the OHSS group (N=150) and the non-OHSS group (N=188). Data were obtained and examined by evaluating the files in a questionnaire. Results: The two groups did not differ significantly with regard to their mean age (p = 0.57). There was also no statistically significant difference between the OHSS and non-OHSS groups regarding the quality of frozen embryo transferred (P = 0.17). Also, there was also no statistically significant difference between the two groups in terms of their pregnancy rate (OHSS = 30.0% vs. non-OHSS = 25.0%) and miscarriage rate (OHSS = 31.11% vs. non-OHSS = 24.44%) (p = 0.32 and p = 0.31, respectively). Conclusion: Ovum exposure to high estrogen during ovulation stimulation does not affect embryo implantation and miscarriage in patients with OHSS.

Open treatment options in massive rotator cuff tears associated with acromion non-union

We evaluated the available English literature for cases of acromion non-union and rotator cuff tear, looking at surgical methods of treatment as well as at functional results. As expected, the literature regarding rotator cuff pathology was found to be abundant, and a good number of articles were found regarding acromion non-union, but articles concerning the association of the two were scarce. Treatment recommendations varied, and were made on a case-to-case fashion. The accepted treatment methods for acromion non-union consist of non-surgical management, open or arthroscopic fragment removal, open or arthroscopic reduction and fixation (bone grafting may be used), while the rotator cuff witll be reinserted with anchors. In this rare occurence, a large rotator cuff tear with retracted tendons combined with an acromion non-union are usually repaired by an open technique in an one-stage surgery, with good results. In order to chose the right treatment option scapular kinematics must be taken into consideration especially when acromial fractures are associated with rotator cuff tears.

Medicinal plants effective in the treatment of glaucoma from the perspective of traditional Persian medicine

Traditional Persian medicine (TPM) is one of the richest medical schools in the world, which has nurtured valuable books. From Ibn Sina's law to Heravi's successful medicinal facts, all emphasize the treatment of various diseases, especially eye diseases based on herbal medicines, as well as their prevention. In this study, by reviewing the authoritative books of Traditional Persian Medicine, we are looking for medicinal plants so that they can be used in the treatment of glaucoma or glaucoma. the purpose of this study is not to express the complete cure of glaucoma, but this article seeks to classify plants that from the perspective of Traditional Persian Medicine can be used as complementary medicine alongside conventional medical treatments. Methods and Materials: This is a summative qualitative content analysis that focused on Traditional Persian Medicine (TPM) texts from 2th to 13th AH centuries. Literature was searched during centuries 4th to 13th AH, and after described of glaucoma, extracting and classifying plants. Results: The scholars of TPM have opinions in the field of recognizing, describing and treating eye diseases and defining practical words. In almost all books of general Persian PM, the diagnosis and treatment of eye diseases have been discussed. Glaucoma in Traditional Persian Medicine is called “black water”. Conclusion: Unlike other medical schools in the world, in addition to treating diseases, Persian medicine has a strong recommendation to prevent the occurrence of disease and also to strengthen the body's visual powers by using 6 principles of maintaining health along with herbal medicines.

Evaluation of the effect of drug on physiotherapy methods on cytokines and chemokines of Covid-19 patients

Previous investigations have demonstrated that interleukin-6 (IL-6), C reactive protein (CRP), tumor necrosis factor- alpha (TNF-alpha), in Coronavirus disease 2019 (COVID-19) patients are considerably increased, and their progressive elevation are clinical threat indicators of disease severity. The purpose of this systematic review is to evaluate the efficacy of physiotherapy, specifically chest physiotherapy, on different cytokines in Covid-19 and non-covid-19 patients. The main complications and symptoms of this virus are as follows; a dry cough, fever, and progressive dyspnea. Quickly, the coronavirus, which is named SARS-CoV-21, has spread worldwide, causing severe lung inflammation, respiratory distress syndrome, cardiac and renal injury, especially in comorbidities patients. Approximately 96% of the cases experience mild respiratory symptoms; some progress to pneumonia, respiratory insufficiency, acute respiratory distress syndrome, and multiorgan failure. The overall mortality rate per number of diagnosed cases is 4.6%; it can range from 0.2% to 15% according to age and health problems. Combination MeSH and text terms were used to perform the search strategy. Interventions in RCTs and clinical trials with or without comparison were assessed. Six studies met the inclusion criteria. Studies demonstrated that physiotherapy could have an effect on TNF-alpha, IL-6, IL-10, IL-1β and CRP.

Introducing a new medical method innovative suture technique to reduce scars in central lip lift medical surgery

The central lip lift surgery with a new suture technique called; continuous locking suture in three cases is reported in this article. In this novel method, we used a 5.0 chromic suture by applying a 30-degree angle of the entrance to the skin semi-deeply. We start suturing at the middle of the wound,3-4 mm from the edges of the skin. The needle should be way out at the exact point of entrance, and this process is repeated to reach out to the two sides of the wound. More than 90% of patients (in more than 100 cases) were satisfied with the cosmetic results. Moreover, 90% of cases did not demonstrate any scar after three months without laser or reconstructive ointments. We evaluate many patients who undergo central lip lift surgery with this novel technique, and the scar problem was resolved in more than 90 % of cases with no supportive treatment.

Evaluation of the effects of dexamethasone in modulating breathing pattern decomplexification in rats with 2-chloroethyl ethyl sulfide induced lung injury

Background and Objectives: Sulfur mustard is a functional alkylating chemical warfare agent that gives rise to appalling lung injury. In people with pulmonary diseases, including asthma, the complication of respirational dynamics is reduced. However, the complexity of breathing patterns in lung injury caused by chemical agents is not clear. In the current study, the outcome of 2-chloroethyl ethyl sulfide (CEES), and mustard analogue, upon breathing pattern of rats without or with treatment were reviewed. Methods: The interbeat interval (IBI) and respiratory volume (RV) data have been acquired from spontaneous respiration rats with lung injury by CEES using a whole-body plethysmograph. We calculated mean and coefficient of variation, alpha exponent derived from detrended fluctuation analysis (DFA), and sample entropy of IBI and RV. Finding: Entropy examination of respiratory variation displayed reduced inconsistency (less complication) in the breathing pattern of this rat model of lung injury. The mustard analogue also led to increased lung inflammation in damaged rats. However, treatment by NAC and dexamethasone had a compelling impact on the complication of the breathing rhythm and lung inflammation of rats with lung injury. Conclusion: Our findings show that inflammation could be the possible origin of respiratory dynamics shifting apart from the normal variation in CEES-induced lung injury.

Evaluation of children with ADHD and their psychiatric symptoms

Background: Mental disorders, especially in children, can be manifested in different ways, which is of great importance to them, so the purpose of this evaluation was to evaluate the changes in psychiatric symptoms in children with ADHD. Method: This evaluation was performed as a descriptive-analytic study on 141 children with ADHD who were followed up for 6 months. At the beginning of the study and the third and sixth months after, patients' symptoms were assessed with the CBCL Behavior Checklist and the ADHD rating scale questionnaire, as well as the course of illness and association with other disorders. Finally, the data were entered into SPSS statistical software and analyzed. Results: The mean age of the children was 5.89±0.81 years and the prevalence of males was 68.8%. Among the two treatment groups, only the mean score below the ADHD-RS questionnaire score in the non-Ritalin group at the end of the third and sixth months was significantly lower than the Ritalin group. During the study in both treatment groups, the overall ADHD-RS questionnaire and both subscales of attention deficit hyperactivity disorder (ADHD) had a significant decrease from baseline to the end of the sixth month. In relation to the CBCL behavioral checklist, the mean score of the total checklist except for anxiety/depression in the non-Ritalin group and the somatic complaints subscale in the Ritalin group decreased significantly from baseline to the end of the sixth month. Conclusion: Based on the results observed in this evaluation, Ritalin had a significant effect on the symptoms of patients with ADHD, although these symptoms improved in both groups, however, further studies are needed.

Climbing – between athletic performance and psychological performance

The aim of this research is to investigate different aspects of the functioning of the psychological system of performance climbers in Romania. We look for correlations between the concepts of competitive anxiety, self-esteem, motivation, willingness to take sports risks and age, sport experience and competitive experience, performance level. We also try to find out if the first contacts with the climbing world resulted in a favouring of the environment in which the athlete chooses his/her performance goals. Specifically, we aim to divide climbers into two categories, those with a passion for panel climbing (indoor) and those with a passion for rock climbing (outdoor). We did not distinguish between the bouldering challenge and the difficult challenge. In addition, we want to analyse the level of competitive anxiety of athletes in national climbing in order to objectify again the idea that the coach and sport psychologist must be able to identify, in the crucial moments before and during the competition, which athletes need motivation, which ones are anxious, which ones feel good and which ones can be left alone. We can classify athletes who need relaxation, activation or mindfulness sessions. Objectives: • Identifying the performance level of Romanian climbing athletes at the moment; • Highlighting the motivational dimensions of performance climbers in relation to the practice of the sport; • Investigating the passion for indoor versus outdoor climbing, with implications on the level of intrinsic and extrinsic motivation and self-esteem; • Knowing the level of competitive anxiety, with a focus on the fine delineation between being debilitating or facilitating for athletes. Methodology: The research method used was the application of instruments (psychological tests and an opinion questionnaire). The test was administered between January-March 2020, online via google docs, on 28 performance climbers, aged between 13 and 48 years, female and male, juniors and seniors. Results: The climber's personality shines through on the rock (outdoors), where the desirability, and if it exists, is not a social one, but is a genuine desire to push your own limits, to face difficult challenges, to feel danger, to develop your wits and to know where to stop. These concepts correspond entirely to intrinsic motivation and a more mature personality, both in social and sporting terms, from this maturity a fundamental dimension of which is the match between real possibilities and aspirations. Conclusion: Rock climbing involves bringing out the authentic self when you are with yourself, compared to a sports competition held at the panel that highlights a socially constructed self, adapted to the fans, an image and smiles to support your image in the sporting world. In other words, avid rock athletes will train at the panel with low anxiety looking at these workouts only as technical drills for the future challenges they will face at the cliff. They are less reliant on extrinsic motivation when conducting their training on the panel, keeping it somewhere in the background for what they consider their own goals. For those for whom the stakes are to increase the difficulty of their rock routes, those will be the situations where they will have higher anxiety and a higher need to rely on extrinsic motivation as well until they reach the maturity where performance is staked by intrinsic determination.

Retinal migraine, a contentious clinical entity

Retinal migraine (ophthalmic migraine, ocular migraine, anterior visual pathway migraine) is a rarely encountered medical condition, but with an interesting and complex clinical presentation. The goal of this article is to compile the literature on retinal migraine, detailing nomenclature, pathophysiology, differential diagnosis, and medical investigations including newer imaging modalities, like optical coherence tomography (OCT). We will approach topics of great concern such as risk factors, complications, treatment, and management of this condition. Retinal migraine is mainly a diagnosis of exclusion and it must be isolated from transient monocular vision loss (TMVL) developed from other causes, for instance, papilledema, giant cell arteritis, carotid artery disease (amaurosis fugax), intermittent angle closure glaucoma, and others. Its treatment is similar to migraine treatment; topiramate, amitriptyline, and nortriptyline are some of the pharmacological drugs successfully used.
