Chronic venous disease has been pathology of great interest over time. With its large prevalence among the world population (10-33% adult woman and 10-20% of men) through profound socio-professional implications that affect the quality of life of patients, chronic venous disease is a real public health problem, being an important cause of morbidity and disability, a permanent area of research in the medical-pharmaceutical field. Special attention is now given to identifying the mechanisms and stages of disease development. From studies that have highlighted the importance of venous hypertension as an essential element in the development of the disease, to this day it has come up to demonstrating the fundamental role of the venous endothelium and glycocalyx in triggering and progressing of chronic venous disease. Biochemical, immunohistochemical, and functional investigations have allowed the evaluation of changes that occur in cellular metabolism and evidence of some immune-vascular factors responsible for vascular alterations in chronic venous disease: specialized receptors, adhesion molecules, cytokines, matrix and metalloproteinases. The analysis of the pathophysiology of chronic venous disease is still a vast area open to scientific research.
Evaluated efficacy of Respiratory Physiotherapy and Remdesivir on patients with COVID-19 pneumonia. In current systematic review and meta-analysis study, articles published January 2019 to December 1, 2021 were reviewed in the databases of PubMed, Scopus, Web of Science, and EBSCO. Risk ratio and mean differences with 95% confidence interval (CI), fixed effect model and Mantel–Haenszel or Inverse-variance formula were calculated. The Meta analysis have been evaluated with the statistical software Stata/MP v.16 (The fastest version of Stata). Mean differences of PaO2/FiO2 ratio at 6h after chest Respiratory Physiotherapy was (MD, 66 mmHg 95 % CI 64.71 mmHg, 67.28 mmHg; p=0.0007). Risk ratio of recovery rate between experimental and control group was 0.20 (RR, 0.20 95 % CI 0.15, 0.25) with high heterogeneity (I2 =78.84%; p=0.00). Risk ratio of mortality rate between experimental and control group was -0.34 (RR, -0.34 95 % CI -0.65, - 0.03) with low heterogeneity (I2<0%; p=0.51). Based on the findings of meta-analysis, Respiratory Physiotherapy can play an effective role in respiratory therapy and rehabilitation of patients admitted to the ICU with COVID-19. A meta-analysis showed that treatment with Remdesivir could increase the recovery rate, especially in the early days of COVID-19; also reduces the mortality rate.
(1) Background: Human papillomavirus (HPV) E6 and E7 oncoproteins are vital for developing HPV induced penile carcinoma. The viral oncoproteins play a central role in oncogenesis by interacting with several cellular regulatory proteins, such as p16INK4a and p53. Many studies suggest that these proteins showed clinical utility in predicting nodal disease, cancer specific survival, overall survival and even tumor grade. Understanding the molecular mechanism involved in the carcinogenesis of penile cancer could offer biomarkers for disease progression, treatment response and potential targeted therapies; (2) Methods: This paper is a prospective study on a group of 100 patients who underwent prostate surgery during 2013 and 2014 in the Urology Clinic of “Carol Davila” Central Military Emergency University Hospital Bucharest. They were tested for HPV by PCR and IHC (p16) methods; (3) Results: 11 cases (22%) of HPV were found in the cluster of patients tested. PCR and P16 were the HPV diagnostic tests used. In order to determine the consistency of the 2 tests, the Cohen’s kappa coefficient was used at a p level < 0.05. The PCR method had a sensitivity of 81.8% and a specificity of 94.9%. The P16 method had a sensitivity of 63.6% and a specificity of 89.7%.
The results of the conducted research gave the possibility to distinguish the structure of the motivation which was suitable for the distinguished types of motivation of servicemen of the National Guard of Ukraine with different levels of effectiveness of their professional activity including motivation based on public service, stagnation, prosaically, romantic, deficient, and dependent. 2435 male servicemen participated in the research. They equally represented all the operational and territorial reunifications and military units of the National Guard of Ukraine. Every unit equally presented the officers, servicemen under the contract including the participants in hostilities as well as those who did not have such experience. The research was conducted with the help of the methods which were standardized by the Ukrainian language: “Close Questionnaire of Examining the Motivation of Professional Choice Made by Applicants of the Ministry of Internal Affairs”, “Methods of Studying the Motivational Profile of Personality”, and “Motivation Sources Inventory – MSI”. The conducted research posed the possibility to describe not only the determined types of professional motivation of servicemen, but also specific phenomena such as self-motivation of the representatives of a type which was run by public service motivation; “rest on the laurels” for the type of stagnation; problems related to choice-making for the prosocial type for whom profession, family, and friends had equal meaning; the tendency for manipulation to gain access to desirable aspects of professional activity of the type which struggled for adventures; lack of sincere interest in the military profession for the deficient type which could change it for more prospective having the same requests for the candidates; attitude to the profession as if it was a means of satisfaction of the needs of their emotional life for the dependent type.
The aim of the research was to determine the social and psychological predictors of alcohol-related incidents involving servicemen. The research involved 310 servicemen, who were subjected to disciplinary and administrative penalties within 2016-2021 years for the offence commitment under the influence of alcohol. The control group included 650 servicemen who joined the service within this period. The analysis of psychological features of servicemen was carried out with the help of the following methods: “Determination of Type Accentuation of Character Traits and Temper” Questionnaire, “Self-Esteem Structures of Temper Questionnaire”, “Multilevel Personality ‘Adaptability’ Questionnaire”, “Progressive Matrix”, “16 PF Questionnaire”, and “Self-Esteem Structures of Temper Questionnaire”. Cluster analysis was used to differentiate servicemen who committed incidents under the influence of alcohol. The determined typology was valuable for the organization of preventive works, events dedicated to the improvement of socialization by recruits, adequate formation of servicemen image as well as their identification in accordance with it, and events dedicated to the prevention of suicidal behaviour.
Although multiple sclerosis (MS) is the most common neurological disease of young adults, afflicting hundreds of thousands of people worldwide, its pathogenesis is still only incompletely understood. There seems to be substantial heterogeneity in disease mechanisms, but in the majority of cases an autoimmune origin or at least a decisive autoimmune component is postulated. Therefore, pathogenic research focuses on different players programmed by the immune system. In this study, biomarkers such as Human myxovirus resistance protein A (MxA), Anti-EBNA antibody and Interleukin S10-12- 17 have been tested. Study of hypotheses show that: Interleukin levels in the blood of MS patients in the experimental group (patients with recurrence) is higher than controls (patients without recurrence condition). The level of anti-EBNA antibody in MS patients in other groups (patients with recurrent) and control group (patients without recurrence condition) is high. The level of the human myxovirus resistance protein A (MxA) in patients with multiple sclerosis in the experimental group (patients with recurrence) is less than the control group (patients without recurrence condition). The level of anti-EBNA antibody in MS patients in other groups (patients with recurrent) and control group (patients without recurrence condition) is high. Finally, there is a significant relationship between viral infection diseases and recurrent multiple sclerosis.
Background and Aim: One of the most effective ways to improve efficiency in organizations is human resources management. Job Anchors and Organizational Commitment are constructs having important implications for human resources management practices, especially in efficiency. The purpose of this perusal was to study a structural equation model (SEM) of the impact of job anchors and organizational commitment on efficiency in a military hospital. Methods: The statistical population of this study was the personnel of a military hospital, which gathered accomplishing simple random sampling method. As a result, the research used two hundred and thirty questionnaires so to analyse, accomplishing as survey was developmental-applied, that was, requirement data with questionnaires. To measure efficiency, organizational commitment and job anchors variables respectively the Persian version of Hersey & Goldsmith, Meyer & Allen, and Schein questionnaires standard were used. In order to test the research conceptual model, structural equations modelling based on partial least squares method was used, and the model was analysed using AMOS statistical software. Results: The results of data analysis demonstrated that hidden variables such as organizational commitment, job anchors, and efficiency had acceptable convergent validity, and the fitness of the conceptual model was confirmed. Based on the research model analysis, organizational commitment did not have impact on efficiency, and job anchors had a positive and significant impact on efficiency. Eventually, the modified model showed that organizational commitment variable moderates the connection between job anchors and efficiency. Conclusion: In light of the findings of this research, managers of military organizations should develop programs to create and improve organizational commitment in personnel. Furthermore, they should pay attention to employees' characteristics and the proportion of their occupation with job anchors in cases of changing and promoting employees. Implementing aforementioned items will have a positive impact on improving the efficiency of military organizations.
This study aims to compare the quality of life in patients with rectal cancer between sphincter preservation and permanent colostomy. The present study is a cross-sectional study that was performed on 120 patients (60 cases in the sphincter maintenance group and 60 cases in the permanent colostomy group) who underwent surgery from February 2017 to November 2016. Quality of life assessment was provided with QLQ-C30 and QLQ-CR29 questionnaires. Finally, the collected data were analyzed using SPSS23 software. According to the results of the QLQ-C30 questionnaire, there was no statistically significant difference in patients' quality of life in terms of performance in the study groups (P˃0.05). Symptoms such as nausea, pain, shortness of breath, sleep disturbance, loss of appetite, constipation and economic problems in the study groups were not statistically significant (P˃0.05). But the mean symptoms of fatigue (P=0.038) and diarrhea (P=0.037) were significantly higher in the sphincter maintenance group than the permanent colostomy group. The mean score of patients' quality of life was 53.43 ± 8.40 in the sphincter retention group and 52.23 ± 11.45 in the permanent colostomy group (P = 0.556). According to the results, the quality of life in the two surgical methods of sphincter preservation and permanent colostomy was not statistically significant. Therefore, treatment decisions should be made by informing patients, both based on their preferences and the physician's clinical judgment.
Leiomyosarcoma is a rare type of smooth muscle cell tumor, with an incidence of 7-10% compared to other types of sarcomas. It develops primarily in the retroperitoneal cavity, the genital tract and extremities. Leiomyosarcoma can also affect the bone, either as a primary tumor or as metastasis. Primary osseus leiomyosarcoma is a rare finding in patients with radiologically identified osteolytic lesions having an incidence of <1% of total primary bone tumors.Due to its low incidence patients with primary osseous leiomyosarcoma are never diagnosed before immunohistochemical analysis and case management is difficult because of the lack of specific indications pertaining to primary osseus leiomyosarcoma. Data on age specific incidence is scarce but there seems to be a tendency towards a higher incidence with advanced age. The purpose of this work is to discuss the complete management of primary osseus leiomyosarcomas based on interdisciplinary medical teams’ indications in a Romanian hospital setting.
Background: NT-proB-type Natriuretic Peptide (BNP) (NT-pro BNP) is one of the biomarkers that has been studied in recent decades in connection with pediatric heart failure (HF) and congenital heart disease (CHD). Recently, infants hypothesized that serum. NT-pro BNP levels might be a good predictor of septicemia severity and response to treatment and prognosis in these patients. This study investigated the relationship between serum natriuretic type B peptide level and neonatal sepsis. Materials and Method: In this case-control study, all full-term and pre-term neonates admitted to children’s Hospital, Tabriz- Iran(2021), with sepsis and a septicemia diagnosis were confirmed after obtaining consent. The study was deliberately entered by the patient's parents or legal guardian. Complete Blood Count with Differential (CBC/Diff ), C - reactive protein (CRP) , Bacterial Culture (B/C) , Urine Analyze(U/A),Urine Culture (U/C) ,NT-pro BNP were checked and finally compared with laboratory results of the same number of infants who did not have clinical and laboratory symptoms of sepsis. After collecting samples, laboratory results, including serum levels of NT-pro BNP, were compared in case and control groups. Results: One hundred patients were studied in two groups. Thirty-eight patients (38%) were boys, and 62 patients (62%) were girls. The mean age of the patients was 7.58±7.46 days. The mean weight of the studied patients was 2811.80±620.33 grams, with a median of 2855 grams. The most common clinical symptom observed in patients in the case group was fever (100%) followed by Poor feeding (84%). In the control group, all patients had jaundice. Neonates with sepsis had significantly higher initial pro-BNP values than the control group (10023.80 vs. 2247.20; p=0.001). The NT-pro BNP level cut-off point in predicting the final treatment status and mortality of neonates with sepsis was 9583 pg/ml with 97.7% sensitivity and 93.6% specificity. Conclusion: Measurement of serum level of NT-Pro BNP in neonates with sepsis at the time of clinical signs with 97.7% sensitivity and 93.6% specificity with a cut-off point of 9583 is an important prognostic factor in the therapeutic management of patients.
