Posttraumatic stress disorder (PTSD) is a psychological-psychiatric state caused by exposure to stressful, traumatic events that manifests as a vivid reminiscence of them by flashbacks, nightmares, reccurent memories, emotional and physical manifestations. Cardiovascular diseases (CVD) are of interest in patients suffering from PTSD because there is an increasing body of evidence that these pathologies are linked. Hypertension, dyslipidemia, diabetes, and other cardiovascular risk factors are common in patients with PTSD and all of them increase the incidence of coronary syndromes, both chronic and acute. Posttraumatic stress disorder has a major impact on the lifestyle and health of subjects exposed to trauma or stress. Cardiovascular diseases tend to appear sooner in PTSD diagnosed patients due to a variety of reasons: poor habits, low addressability to health care systems, chronic inflammation status, changes at molecular levels, etc. Otherwise, PTSD tends to be induced by CVD, thus inclining the balance towards whis association. According with data published until now, there is a strong pathophysiologic relatonship between PTSD and some CVD; there is also outlining a vice versa relationship, from some CVD to PTSD.
In accordance with the results of conducted research there were figured out the content, hierarchy, intensity of motives and their possibility to be implemented in servicemen with various levels of effectiveness of service activity. 2435 male servicemen participated in the research, they equally presented all the operational and territorial military unifications and units of the National Guard of Ukraine. Each unit proportionately presented officers as well as military personnel under contract who included the participants of hostilities and those who did not have such experience. Examination of various aspects of servicemen motivation was conducted with the help of the following the methods which were standardized being based on the Ukrainian selection “Close Questionnaire of Examining the Motivation of Professional Choice Made by Applicants of the Ministry of Internal Affairs”, “Questionnaire of Estimation of the Effectiveness of Military Tasks’ Execution by Servicemen of the National Guard of Ukraine”, questionnaire on “Sense-Bearing and Life Orientations”, the methods of “Studying the Motivational Profile of Personality”, and “Locus of Control”. There was conducted the cluster analysis which gave the possibility to figure out six motivational types connected with the effectiveness of servicemen operational activity. Motivational types were located between the poles which were introduced by two dimensions: self-efficacy (autonomous and controlled motivation) and motivation of helping others (motivations of public service - selfishness). Those motivational types of servicemen were the following: motivated by public service, stagnant, prosocial, romantic, deficient, and dependent. Distinguished types included the content, hierarchy, intensity of motives and their possibility to be implemented
Multiple sclerosis is a chronic autoimmune disease, characterized by the appearance of demyelinating lesions with inflammatory character in the CNS. Given the unpredictable evolution of the disease, numerous attempts have been initiated to find some pathogenic factors and by default targeted treatment methods. The intestinal microbiota of these patients is often significantly altered compared to the healthy population and given the complex connections between the intestine and the central nervous system, the idea of causality between dysbiosis and this condition was launched. The intestinal mucosa represents the interface between the external environment and the organism, so that it is considered the starting point of some local immunological processes but with systemic resonance, especially neurological. The classic medication scheme widely used to treat MS nowadays can also affect the integrity of the intestinal mucosa by breaking the fine balance at this level. Data from recent studies around the subject open new treatment opportunities around preserving the intestinal microbiota and strategies like vitamin A intake and fat-soluble vitamin D supplementation, probiotic complexes or faecalis microbiota transplantation appear more promising and easier to apply in daily clinical practice.
Background: Rheumatic disorders are one of the most prevalent human diseases which commonly become chronic and cause different types of disabilities. Chemical veterans may suffer from chronic difficulties caused by rheumatic disorders that can lead to decrease of their quality of life. The main purpose of this study was to evaluate the prevalence of rheumatic disorders among Iranian chemical warfare patient’s long-years after sulfur mustard (SM) exposure. Methods: This cross-sectional study was conducted from 2013 to 2014 on 245 Iranian chemical veterans. In order to evaluate rheumatic disorders, a special questionnaire for COPCORD studies were filled by veterans and then general and special rheumatology examinations were applied under the supervision of internists and rheumatologists. Data from all questionnaires were analyzed by SPSS software and the results were reported as percentage or mean ±SD. Results: Osteoarthritis (69.9%) was the most common complications diagnosed in these patients. After that back pain (30.5%) and pre-arthritis (27.2) were the other popular complications among veterans. The percentage of frozen shoulder and moderate carpal tunnel syndrome was 13.2% and 11.5%, respectively. Tennis elbow and fibromyalgia (3.7%), rheumatoid arthritis (0.8%) and gouty arthritis with 0.4% were the lowest complications among these patients. Conclusion: Rheumatic associated complications in Iranian chemical injured veterans are higher than the mean of whole community. This data provided the possibility of calculating the disease’s burden and making efficient decisions in the sanitary and treatment systems.
Introduction: Complication after inguinal hernia repair is one of the challenging problems after inguinal hernia repair. This study aimed to compare the complications of three common surgical methods in inguinal hernia repair, Bassini, Lichtenstein, and Preperitoneal methods (with anterior approach). Methods: In this prospective study, patients who underwent inguinal hernia repair surgery in Baqiyatallah Hospital from June 2019 to May 2020 were included. Patients were allocated in three groups: Bassini, Lichtenstein, and Preperitoneal (with anterior approach), based on surgeons' opinions. Postoperative pain, hematoma and seroma, wound infection, recurrence, and also we assed postoperative satisfaction after surgery. Results: The mean age of patients was 49.90 ± 16.44 years, and 92.5% of patients were male. Of the total patients, 6.8% had an infection and there was no significant difference in the distribution of infection in the groups (P=0.230). The mean of pain in all patients was 2.37 ± 1.10. There was no significant difference between the three groups in the mean of pain (P = 0.253). There was no significant difference in the distribution of recurrence in the groups (P=0.356). There was no significant difference in the distribution of testicular swelling in the groups (P=0.421). No postoperative hematoma and seroma were reported in the groups. The mean of satisfaction in all patients was 93.61±19.63. There was no significant difference between the three groups in the mean of satisfaction (P = 0.156). Conclusion: The results showed that infection, pain, and recurrence rate in the Preperitoneal group were lower than the other groups, although this difference was not significant. Also, testicular swelling, hematoma, and seroma as well as patients’ satisfaction were similar between groups.
Background: The Beliefs About Obese Persons Scale (BAOP) is used among non-obese people in English speaking countries to evaluate their opinions on obese people. Nevertheless, the data are scarce from the view of obese people. Therefore, we wanted to test BAOP on obese patients in order to evaluate their opinions from the perspective of obese patients. Methods: Between August 2019-June 2020, 115 (76.7%) female and 35 (23.3%) male, a total of 150 obese patients who were admitted to the general surgery department to be evaluated for bariatric surgey, were included in this study. Local ethics committee approval was obtained (2019/08-10). These obese patients were asked to fill out BAOP, which included 8 statements in which each statement was valued between -3 to +3 points. Results: Mean score of BAOP within all obese patients was 34.87 ± 8.12 (range: 5-48). The average score of BAOP was 34.33 ± 8.60 (range: 5-48) in females and 36.62 ± 6.06 (range: 21-48) in males (p = 0.14). The frequency of the answers given by the male and female patients to the 5th statement, which was “Most obese people eat more than non-obese people”, showed a statistically significant difference (p = 0.03). Conclusions: Obese patients had higher BAOP scores compared to previous BAOP studies performed with non-obese people. Within this study, obese patients considered obesity as a condition which was not under their control.
Vulvovaginal atrophy is a condition frequently encoutered among menopausal women that often is ignored, remains undiscussed with the physician and the patient adapts the life style is such matter that offers a run from the specific symptomathology. According to the specialized published data, only 25% of the patients with vulvovaginal atrophy receive adequate therapy. The therapeutic perspective is quite extensive but the patient should receive recommendations and try different products until they find one that meets their needs and the one they like best. Proper estrogen therapy leads to a restoration of the acidic, normal vaginal pH, of the vaginal microflora, thickens the epithelium, increases vaginal lubrication and decreases vaginal dryness, also, adverse effects of vaginal estrogen therapy are uncommon. Early active detection of this pathological condition and the provision of an appropriately selected therapy can prevent the progression of the symptomatology and can significantly improve the sexuality of women.

After primary care systems administratiοn, general practitiοners acquired a special rοle in the οperatiοn and efficiency οf these systems. Their existence as institutiοn cοntributes tο the mοst cοmplete and better preventiοn and treatment οf health issues, as well as tο the resοurces saving fοr the insurance systems. In Greece, since the fοrmatiοn οf the Greek Natiοnal Health System in 1983, numerοus interventiοns have been legislated, mοst οf them referring tο the institutiοn οf family dοctοr. Hοwever, factοrs related tο pοlitical will, financial interests, administrative and educatiοnal inadequacies, financial prοblems and geοgraphical particularities did nοt allοw the establishment οf a strοng primary healthcare. CΟVID- 19 οutbreak has expοsed the weaknesses οf all types οf health systems. Hοwever, health systems based οn primary care seem tο better cοpe with the pandemic, as well as the cοntinuity οf care and quality οf health services have been secured. In οrder, that Greek primary healthcare be οrganized frοm nοw οn, many actiοns are required such as the establishment οf electrοnic health recοrds, the creatiοn οf prοperly staffed and equipped health centers, as well as mοre gοvernmental financing prοgrams abοut the mοdernizatiοn οf primary care. Last but nοt least, pressure shοuld alsο be exerted οn gοvernments tο pass mοre grοundbreaking bills and implement sοme οf the οlder prοvisiοns.
Magnesium alloys represent a new research direction in the field of orthopedic resorbable metal materials, because it has several important properties that recommend it as an excellent resorbable biomaterial for implants. From in vitro studies, Magnesium has osteoconductive properties, promoting the formation of new bone by stimulating the adhesion of osteoblastic cells, this leading to the idea that it will induce formation of hard callous at fracture site. The present study followed the in vivo evaluation of the biodegradation of pins made of Mg-1Ca alloy introduced centromedulary in rabbit tibiae, with previously induced experimental fractures. The influence of the biodegradation process on the formation of the bone callus was evaluated through radiological images and histopathological examinations. At the same time, we evaluated the function of vital organs through histopathological examinations and blood tests. We chose intramedulary nailing (IMN) with pins, because in orthopaedic surgery this is the golden standard for fractures of tibia or femur. In the radiological images obtained, we notice callus formation at the level of the experimental fracture at 6 weeks after surgery. The histological sections made from the samples collected from the fracture zone adjacent to the Mg1Ca alloy implant, confirm the formation of the bony callus. The trichrome Masson sections show a normal bone development with newly formed bone tissue at the tissue - alloy interface. In the sections harvested from the vital organs, no pathological changes are observed. These observations suggest that the biodegradation process of the Mg-1Ca alloy do not interfere negatively with the biological process of bone formation, allowing the formation of the bone callus and do not confirm the suspicion of pathological changes induced by systemically released corrosion products and gas embolism in vital organs.
Diabetes and increased body mass index are two of the most associated comorbidities in patients undergoing total knee arthroplasty. With the number of patients undergoing knee arthroplasty increasing and with the incidence of diabetes and increased BMI, we can expect that in the coming decades the association of the two comorbidities will be frequent. An ethical issue arises when the patients who are refused for total knee arthroplasty are instructed to change the risk factors, and they fail either because of a mental problem or because of a physical problem. This study analyzes the effects of controlled, uncontrolled, uncomplicated and complicated diabetes on the rates of post- knee arthroplasty complications. The study proposes to evaluate obese patients in terms of functional outcomes and postoperative complications after total knee arthroplasty.
